All Patients

Please bring any relevant imaging or test results such as X-rays or Ultrasounds

Privately Insured or Self Funded

No referral is required. However, some health insurers request a medical referral.

Please contact your private health insurance fund to check if you have the appropriate level of Extras cover ‘Occupational Therapy’ extras to obtain a rebate following account payment.

Please note that a Medicate rebate is NOT available for this service unless your GP has decided that you qualify for a Chronic Disease Management Plan (previously known as an Enhanced Care Plan).

Medicare Chronic Disease Management Plan

If your GP has given you a Chronic Disease Management Plan referral to bring, payment is still required on the day of treatment and any therapy items provided. The Medicare rebate will be processed on your behalf directly to you from Medicare.

WorkSafe, TAC or Department Veteran Affairs

Worksafe, TAC and Department of Veteran Affairs patients all require a treating doctor or surgeon referral and need to bring their claim numbers and Worksafe employer details.
